THE ROLE OF PHYSIOTHERAPY IN COVID-19 RECOVERY

  • The experience of Prim. MUDr. Cibulka Lukáš (Rehabilitační oddělení Oblastní Nemocnice Kolín a.s.) highlights that the daily application of Rib Cage Protocol for 5 consecutive days significantly improves the clinical condition of the patient.
  • The patient is therefore able to quickly begin a rehabilitation program based on ASA Global Approach.
  • To promote the recovery process, an individualized rehabilitation program is often necessary – consisting of modular exercises on individual subjects – and has to be adapted on patient’s needs based on both initial clinical evaluation and progresses during the recovery phases.

Rib cage protocol

Positioning: patient is in prone position with hands under the head for maximal scapular protraction to reduce the muscle and bone barrier and improve laser penetration from posterior thorax;

Parameters

Treatment modality: Scan Rib Cage, from apex to base

Frequency: 1.500 Hz

Intensity: 100 %

Time: ~14 min x 2 (will vary following the Area)

Area: ~ 250 cm2 x 2 (may vary based on patient Size)

Target Dosage: 7,1-7,2 J/cm2

Following the Global Approach principle, MLS® allows the treatment of large target areas and of different areas at the same time, stimulates cell metabolism and myogenesis, eliminating pain symptoms.

The recovery / treatment plan can be applied to different topics:

  • Rib cage contracture (i.e. pectoralis major, sternocleidomastoid, rectus abdominis, intercostal);
  • Muscle repowering – inferior and superior limbs, especially hand and quadriceps;
  • Reinforcement and recovery of the standing position through myofascial stimulation;
  • Pain, both neuropathic and nociceptive.

Above mentioned targets must be subject to professional evaluation in order to adapt the rehabilitation path of the patient based on single needs.

Evaluate what is the MOST RELEVANT CONDITION to treat:

  1. Contracture in the muscles around the rib cage
  2. Weakness at inferior/superior limbs
  3. Fatigue in keeping upright position
  4. Muscle/Joint Pain

1. Contracture in the muscles around the rib cage

  • Breathing exercises and chest enlargement exercises, reconditioning of the respiratory muscles subjected to the stress of prolonged cough and treatment of possible pain contractures of the thoraco-abdominal muscles;
  • Relaxation exercises;
  • MLS® Laser Therapy for pain due to contracture;
  • Treatment of contracted muscles through scanning robotized head.

2. MUSCLE REPOWERING (inf and sup limbs, especially hand and quadriceps);

  • Careful exercises of muscle recovery and stretching of upper limb (shoulder, elbow, hand) and lower limb muscles;
  • MLS® Laser Therapy for pain and biostimulation of muscle weakness.

Treat with multidiode head using standard Biostimulating setting.

3. REINFORCEMENT AND RECOVERY OF THE STANDING POSITION (myofascial stimulation)

  • Manual methods of stimulation of the muscle-tendon bands;
  • Scanning with MLS® Laser Therapy at distance, in particular of the posterior fascia (important indirect photomechanical effect on fibrocytes and Extra Cellular Matrix – ECM);
  • Bilateral quadriceps and triceps sural stimulation.

Treat with multidiode head using standard Biostimulating setting.

4. PAIN, BOTH NEUROPATHIC AND NOCICEPTIVE

  • Dermatomes, myofascial painful areas, joint painful areas (optical windows), Trigger Points (if present).
